Qualified Temporary Services, Inc., has identified you as a person impacted by a data breach incident that occurred on its computer systems between September 28, 2021, and October 13, 2021. You may have already received a Notice of Data Privacy Event about this incident.
A proposed Settlement in this case may affect your rights and may entitle you to a cash payment, as well as credit monitoring and identity-protection services.
Qualified Temporary Services, Inc., (“QTS”) also does business as Qualified Staffing.
A class action lawsuit captioned Michael Lockridge v. Quality Temporary Services, Inc., Case No. 4:22-cv-12086 (“Lawsuit”), was filed in the United States District Court for the Eastern District of Michigan (“Court”), on behalf of everyone identified by QTS as having been impacted by a Data Breach that occurred between September 28, 2021, and October 13, 2021.
QTS denies any wrongdoing and denies the claims and allegations asserted in the Lawsuit. The Court has not ruled on whether QTS did anything wrong. However, the parties have agreed to settle the Lawsuit (“Settlement”) to avoid the risks and uncertainty of continued litigation.
You may receive a cash payment and credit monitoring and identity-protection services if you are a member of the Settlement Class and timely submit a valid claim form.

YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT | |
SUBMIT A CLAIM FORM BY JULY 25, 2024 | This is the only way to receive a cash payment and credit monitoring and identity-protection services. If you submit a Claim Form, you will be bound by the terms of the Settlement. |
EXCLUDE YOURSELF BY JUNE 25, 2024 | You will not receive either a cash payment or credit monitoring and identity-protection services under the Settlement, and you will not be bound by the terms of the Settlement. This is the only option that allows you to ever be part of any other lawsuit against QTS about the legal claims in this case. |
OBJECT TO THE SETTLEMENT BY JUNE 25, 2024 | Write to the Court about why you don’t like the Settlement. |
GO TO A HEARING ON JULY 18, 2024 | Ask to speak in Court about the fairness of the Settlement. |
